Custodian fees for Gold IRA - are these typical?

The IRA itself is around the $750k mark, maybe a little more if gold keeps doing what it's doing.
I guess I'm asking: is this a pretty standard rate these days? I've been in the dairy business my whole life up here in Wisconsin, and you learn quickly that every penny counts. $250 might not seem like much on a $750k portfolio, but that's $250 that isn't compounding for me. I've heard some folks talk about flat fees versus tiered fees, and I'm on a flat fee here. Does it make sense to switch to a tiered structure as the portfolio grows, or is it better to stick with a flat rate?
I've seen some companies advertising lower fees, but then you dig a little and there are all these hidden charges for transfers, statements, or whatever else. I'd rather pay a bit more for transparency than get nickel-and-dimed. Anyone here have experience with different custodians for larger Gold IRAs? Specifically looking for folks who are storing decent amounts, not just a few coins. Are there significant differences in fees around the $500k to $1M mark?
Also, any thoughts on Delaware for storage? I picked it because it seemed like a good, neutral location, and Brink's has a good reputation. But if I switch custodians, should I be thinking about different storage locations too? Just trying to make sure I'm not leaving money on the table or overlooking a better setup.
